Lone Star Rally Hospital Ministry Requirements

Please read completely the following requirements for serving as a LSR Hospital Ministry Volunteer.

There are a few steps you need to complete prior to serving in the hospital ministry.

     First, you need to be available for the Orientation Meeting at the Hospital on Thursday morning October 31. Time of this meeting will be Noon until 1 PM.

     This orientation meeting is critical to your serving in the hospital ministry and will be where you will meet with the hospital staff for your hospital walk-thru and will receive from them your ID badge and parking pass (if a pass is needed).

     If for some reason you are not able to attend the Orientation meeting on Thursday, please indicate so by checking the appropriate box on the signup form, or if you are unsure at this time, let us know as soon as possible so that arrangements can be made for you to receive your orientation later when you arrive at the rally.

     Second, you will need to complete and sign both the Personal Health Information (PHI) form and the Badge Application Form.

     We are working with the hospital currently to produce electronic versions of these forms. As a result, both forms are unavailable at this time but will be forthcoming. If you sign up today, you will be notified via email when the forms are available for you to complete and sign. These forms must be completed no later than October 17th for you to participate in the LSR Hospital Ministry.

     Finally, you are required to provide a photo for the ID badge that you will be given. This photo must be provided to us no later than October 17th.

     The following are some options for providing this photo.

1. If you have served in the hospital ministry in the previous couple of years, we most likely have your photo on file. If you choose this option on the form and we do not have your photo on file, you will be notified via email and you will need to follow option 3 listed below.

2. If you are a chapter officer with a photo on your chapter’s CMAUSA website, choose this option on the signup form and we will use that photo.

3. Have someone take a photo of you with your cell phone and upload the photo using the Hospital Ministry ID badge Photo Upload Form. You can upload your photo using the link provided below.

Please complete and sign the following 2 required documents and bring them with you to the Hospital Orientation Meeting on October 31st at 12:00 Noon.
